Trigger Night with the Thickest Lining on the Planet
It's a dismal cycle.
2 follicles and a cyst on the left. Nothing on the right. I was right: the RE's initial Dx of 9 follicles was way out of line. Do I know how to read an U/S by now or what? H-E-L-L-O????? I can tell the difference between a fluid filled follicle and a blood filled one (endometrioma). Why can't my RE?
Sigh.
And get this. My lining is at 14mm. No, that was not a typo. FOURTEEN. Holy crap. I know it is because I didn't get a full menstrual cycle last AF and so what was left in there has been built upon. I have never had a lining this thick. My gawd. If there is anything to put back in there, it will sure be cozy!

cyst @ 23mm. We doubt that the cyst will have anything in it, but you never know.

So I'm trigger tonight at 3AM - retrieval is on Wednesday at 2:30PM.
I am going to do a 5DT no matter what I have. I know that research is now saying that if an embryo can't make it to day 5, they're simply not viable to begin with. I'm not sure it's true, but enough REs are saying this that I am going with the crowd on this one.
I don't want to endure the 2WW and progesterone shots for nothing. The 2WW is hell. Progesterone shots are hell. So, we'll let them grow and see what we have in the end.
What would you do? Grow them 3 days and stick them back in the oven? Or go for 5 days?

In my deep heart I think you're probably right. About day 5 (or even 6). But I'd get the fert report on day three and see what's there - many people say flagging embies NEED to be in the womb to survive, and I've heard stories of crap embies transferred in despiration on day 3, and they get preggo! (why does this never work for ME???!) I have always done 5 day transfers, but I have no baby to show for it - so take that with a grain of salt, eh?

I've heard some of the same stories, get those embryos into the womb, but I have started to wonder if they're like the story of, "...and then my friends adopted and wouldn't you know it...they got pregnant!" More then exception than the norm?
Dr. Moustache (IVF 1, 2, and FET) out in California said the same thing to me: "The best place for your embryos is your uterus, not our lab" or some such thing.
I should send him a note to see if he still follows that credo before I make up my mind. Luckily I have a few days to make a final decision. I just hate 2WW so much.
I've been thinking about 3dt vs. 5dt a lot, too. Even if I get a decent amount of eggs, we may still end up with just a few embryos to work with since we only have one vial of my DH's sperm left and finding motile sperm in the past has been a challenge.
So yeah, if we only have a few good embryos on day 3 do we still go to blast? I had a cycle where I only had 3 good embryos on day 3 and we just went ahead and transferred them then. I did end up getting pregnant but the embryo never developed a heartbeat. I wonder if the same thing would have happened if we had gone to day 5? Could have I been spared that m/c if the embryo started degrading in the lab? Never know, of course.
This cycle, though, I am thinking day 5 as long as we have more than 2 on day 3. It may end up being a moot point, it may not.
I'm sorry you didn't have a better response. I guess you can always wait and see what things look like on day 3 before deciding. Good luck!!!

It's such a hard decision. My RE in NYC said that what you experienced is typical of 3DT.
Apparently the embryo is dividing on it's own for the first 3 days from the egg's "instructions" - and on day 3 division progressions from instructions from the egg to that of the combined genome of the mother/father. So this is why they say it's good to do a 5DT - you find out if the embryo has the ability to move forward on it's own. Some paraphrase this as saying that the embryo divides the first three days from the mother's egg's energy and "then the sperm kicks in on day 3" but this is an oversimplification of a complex process.
I met with a new doc in NYC who said that doesn't like to do PGD on a 3 day embryo because they haven't "proven themselves" yet.
So I keep hearing stories that propel me to a day 5 transfer...
I have had up to 4 put back in after day 5.. had 1 live birth, so I am not so sure about it at all. I wanted to do 5 on day 3 but got shot down. If I had less then 3, I would go for 3dt.. just cause I am old and stuff...

Thanks for posting your experience. So you did a 5DT with 4 embryos and delivered? Wow. Okay, one more vote for a 5 day transfer!
I have put in 6 at a time (more than once) but all were 3DT. I so wish I knew then what I know now. I would have gone for a 5DT back then.
